H1N1 VACCINE CLINIC AT ARBOR MONTESSORI !
WHEN:
FRIDAY, DECEMBER 18TH, 2009, 9:00AM – 12:00PM

REGISTER FOR YOUR FLU SHOT TODAY!
1. Go to www.flubusters.com
2. Click the red button on the right side where it says, “Register for a flu clinic near you”
3. A new window will open. Where it says, "Type of Clinic" select "H1N1" from the drop down menu.
4. Under "Zip Code" type 30033.
5. Click on "Clinic Search"
6. The clinic will pop up with the date and time. Click on "Select Location".
7. The available time slots will appear. Select the time you would like to be at the clinic. You may also follow the link to view the H1N1 Consent Form. NO VACCINATION WILL BE ADMINISTERED TO ANYONE WHO DOES NOT HAVE A SIGNED CONSENT FORM!!!
8. After choosing a time a box will appear asking you to select a product, select Flu Shot. You will need to do this more than once if you need to order for more people. Example: click 2pm, select Flu Shot, click 2pm again, select Flu Shot
9. After selecting the number of vaccines click "View Cart/Check Out" You will be able to enter names of the recipient(s), phone number and e-mail address. At that time you may also make payment by credit/debit card, or Medicare.
THERE WILL BE AN ADMINISTRATIVE FEE OF $20 PER SHOT FOR THE H1N1 VACCINE, WHICH SHOULD BE REIMBURSED BY YOUR INSURANCE PROVIDER!

Arbor is a unique school. Arbor was founded in 1970, growing from a small one-room class to four primary classes, five elementary classes and a middle school. Arbor has two campuses: La Vista (2998 La Vista Road), and Emory (1886 North Decatur Road.) We are an AMI Accredited school. As an independent school, Arbor follows a traditional school year (usually August through May) calendar. Arbor Montessori School Inc. is a non-profit private school directed by a Board of Trustees composed of parents, teachers, and administrators. Although the two Emory primary classrooms are physically separated from the Central Campus on LaVista Road, Arbor is one school, maintaining the highest standards of Montessori education.
Arbor’s combination of programs, facilities, location, and a dedicated community of parents and staff makes Arbor a unique school. Arbor has a rich history and a vibrant future. We are one of the largest Montessori schools in the southeast and continue to be the flagship of Montessori education in Atlanta.
